APNT, Inc (APNT) partners closely with physicians and hospital case managers to provide high-quality in-home care as a safe and effective alternative to extended hospital stays. Our goal is to help children transition home smoothly while continuing to receive the specialized care they need.

We work hand-in-hand with insurance providers to coordinate cost-effective services tailored to each child’s unique medical needs, ensuring families receive the best possible care without unnecessary stress.

Our team of highly skilled pediatric nurses is experienced in delivering specialized, in-home care for medically fragile children. To further support our families, we also collaborate with trusted independent contractors who have expertise in caring for patients with advanced, high-tech medical needs.

Once a family chooses APNT, our Director of Nursing coordinates with the full healthcare team to develop a personalized plan of care before the child is discharged from the hospital. This ensures a seamless transition from hospital to home.

After the child arrives home, our nurse liaison connects with the physician within 24 hours and maintains ongoing communication to ensure care remains consistent, effective, and aligned with the child’s needs.
